What are the symptoms of a worn 3rd/4th synchroniser ring?

Common symptoms include crunching or grinding when selecting 3rd or 4th gear, especially during quick changes or when cold, and sometimes difficulty holding the gear in position. These symptoms worsen as wear progresses.

Should I replace the gear and synchroniser ring together?
